    Tragedy of Healthcare in America

    Like me someone else (writer of Galen Institute: Grace-Marie Turner) visited the doctors office this week and here is a really interesting anecdote she shared with her readers:

    I visited a busy medical practice this week in Washington, D.C., and saw first hand how frustrating today's third-party driven medicine is for both doctors and patients.

    I had made an appointment to get a baseline bone scan (at the insistence of several of my women friends) and went to the front desk of the 12-person ob-gyn practice to check in.


    The "receptionist" handed me a long paper form to fill out (despite the fact that I am an existing patient and they already have this information) and demanded my insurance card. I told her that it would be of little use to her since I have a $2,000 deductible; this is my first, and probably only, trip to the doctor this year, and I'll be paying cash.


    "Then I have to take your Visa card or a check now. You can't go in until I have payment up front."


    I suggested that this is not the way consumers are accustomed to being treated. We are people, not "billable items."


    A few minutes later a nurse called me in and said she had to take my blood pressure. I said this might not be the best time to do that, but she insisted. (It was 190/90, when it is usually 120/60.)


    I talked with both the doctor and his office manager about my frustrations, and they shared their frustrations with me. They said it is almost impossible to run a practice these days: Patients complain about the way they are treated at the front desk and that they get too little time with the doctor.


    But they said they are paid so little by health plans that they can't hire more experienced office staff, and they have to run patients through the offices rapidly to make their numbers work.


    The office manager was clearly at her wit's end in trying to train staff, (two workers hadn't bothered to show up that day) and she hadn't told the doctors yet that the practice has more than 100 babies due in the month of September alone!


    And doctors are virtually being held prisoner by the system: The office manager said that if a doctor wanted to move to Boston, for example, he would have to buy a "tail" for his malpractice insurance equivalent to three years of premiums (in case of future lawsuits). Plus paying the malpractice insurance in the new city!


    The practice also tried to drop several insurance companies that were paying below costs, but were told they could not drop them. (This is the People's Republic of Washington, D.C.)


    I suggested they might want to consider a new way of dealing with cash customers like me, but the idea clearly instilled fear.


    Physicians have been the first to lose their health care freedom to third-party payers, and it is taking a terrible toll on morale and their ability to freely practice medicine. The doctors who are happiest are those who have moved to cash practices. This is why the incentives and initiatives we support though consumer-directed health care, like funded accounts and portable health insurance, are so important. Despite what Dr. Avorn says, this is the way to get doctors and patients back in charge of medical decisions.

    So what if I cant go out!

    Allright so I have come to terms with my fatigue. Every single day last week, I would get up and feel tired and depressed because I was expecting to be active and hadnt plan to lay around in bed all day feeling like shit! Today I am ready to face this and have planned out my whole weekend.

    Its going to be a movie marathon :-) I went shopping this evening with all the energy I had left and bought:

    Mano ya na mano

    mononucleosis (say: mah-no-noo-klee-o-sus), called mono for short, is caused by the Epstein-Barr virus (EBV), which is a type of herpes (say: hur-peez) virus.

    Also called the Kissing disease is something that I may have which is why I am getting so fatigued. But there is a good chance I may not have this, I have no bacterial infection so anti biotics are out (I felt kinda sad at this point) and so Strep and pneumonia are out. My fatigue is the only scary part which indicates this. Supposedly this continues for months, I just hope I dont have this. The doctor said I am probably towards the end of this and should expect a change by next week.

    The coolest thing was the clinic was using our software!! I could see the Smart Text they used, I could tell me short cuts. They just went live 2 days ago so they were learning it all very quickly. It was fun to watch me being put in as a patient in there, I have put in countless fake patients in the system, so it was fun to be now a part of the whole thing! Also something to talk to everyone in the clinic about, I finally feel like a part of the healthcare world !

    I was going to take the ED test tomorrow but with all this I dont think I can study for it, so I am going to push it to next week. I cancelled Toral and Arpit's visit, with me sulking all day I doubt I can take them around and have a good time. I was trying to go home instead but there are no return tickets available at this point. Looks like I am stuck in Madison. I think I am just going to sleep the whole 2 days. I bought some turmeric, so now I can drink that 3 times a day, will help soothe the throat.

    I exchanged my red solara for a champagne one but this one doesnt have the auto-lock function. That just pisses me off, but I already agreed to change it so I did, besides the other one needed an oil change or something. I guess I will keep this for a while. Maybe change it next time I fill up.

    A rigmarole of stuff that I am doing to get this cold off my head, nothing seems to be helping. Not the nyquil, not the tylenol or tons of fluids, soups, hot water, steam, salt water gargles. Looks like its going to take its usual life cycle. What I am trying is basically making me feel better for a little while, then its back to normal.

    I worked till 2.30 yesterday then got back to the hotel room. Continued to work till 5.30 and then watched an episode of Six Feet Under and another of Sopranos. Both excellent episodes, and made my dreams much more interesting. I think nyquil really messes with your sleep, you end up dreaming even if you dont want to.

    I am going to conquer this

    20+ hours of sleep

    I think I am doing all I can to beat this virus. I had to stay within my hotel room all day today trying to get on the path of recovery. I managed to get my back up to take my customer calls and reply to as many emails as I could. Overall I guess I did around 3 hours of work today. Watched 2 episodes of 6-feet under and managed to do my expense report for last week.

    My throat still itches but its getting better. Just took my 4th dose of Nyquil, hopefully I will be able to get some sleep tonight, having slept all day till 6 pm its going to be rather difficult to sleep all night. I am definately going to work tomorrow, no matter what!

    Feel like the AFLAC duck right now, but I dont want to let them know I am eavesdropping on their conversation.

    My weekend's almost over, sitting at Detroit airport right now I feel completely overwhelmed and under the weather. Its definately the cold air in the house I was saying in columbus last night, those two comforters were clearly not enough. I have been sneezing, sniffling and coughing all day and now I feel like I am going to get sick. The fact that my plane is delayed for an hour does not really help.

    Weeked has been good, lots of family time with my sister (6 hours drive to and fro to Columbus, OH from Detroit, MI) with Jigar and Palak (at Punu aunty's place and at the dinner party last night). Reached Detroit on Friday night, great dinner at Lashish (my fav middle eastern restaurant) in Canton, MI (where they are going to be getting a new IKEA soon!). Night was expecially hard because of my sisters bed, it was rock solid and made me getting up with an aching back in the middle of the night and then sleep on the floor using a comfortor and sleeping bag.

    Reached Dublin around 3. Went to Sher-e-Punjab where they had their dinner reception party. Met a lot of people, among those I remember Nihar, Harshal, Amit, Couple from Cleveland, Punu aunty and Raj Uncle, Liby, Deah, Kirti, motel owners from Athens, OH. Rest of them I wasnt introduced to or have forgotten them already. Lots of socializing without alcohol, feels wierd but good all the same. No late night, got back around 10, watched the pictures from the party and then off to sleep around 11.

    Got up around 8 today, met up with Kedar for brunch at Alladin, another middle eastern place in Dublin. Sat in his awesome G35 which was nice right after Jigar's Acura TL. I think I liked the TL much better when both of them paid the same amount. I love the GPS system in the Acura. Stopped on our way back to Det to an outlet mall, bought my first pair of utility jeans. Should be a good change. I want to get those white shoes now, to go with the jeans. Got to Detroit Airport as planned around 5 and wonderful dinner at Westin with my sister. The creme brulee was awesome as usual, specially the rasberries and blueberries.

    Something new I learnt this weekend:

    A pregnant woman's bulge (and I mean full blown bulge) does not disappear right after delivery, infact it takes around 3 months and much work on the woman's part to get it anywhere close to normal.

    Ok, rest of the song is not that interesting. I just liked this part. I thought of this when Arpit asked me who the girl I met was...

    In any case it wasnt the girl I interviewed. That one was kinda dumb, well she was smart but very poor presentation skills, no introduction, no agenda, no conclusion, no content = no job. Apparently one of the other interviewers said she had a huge problem with travel, not something I found out in the interview. Everything else she seemed to be pretty good with but for a teacher (she was a high school spanish teacher) that was poor presentation. And her topic was 'how to draw a cartoon character'. No powerpoint, which is something I like. I agree with what my professor said, powerpoint tends to paralyze you, if you are confident you can do it without a slide show. Inspite of this, I guess I just want to see things on the board when someone is talking about it. It does tend to paralyze me. Overall I thought it was a good experience for me interviewing, even though I did have a set of questions handed to me to ask her.

    Rest of the day, wasnt too bad. Started off kinda late, around 8 ish. Faxed my tax papers to the HR people in Mas, they told me if I had to pay money because of two state taxes and improper credit from one to the other, then they would compensate me. I hope thats right. Got my agendas ready for the next on site trip end of April. Got some other work done for the more integrated areas of implementation. The highlight of the day was Vin Santos, my favorite italian restaurant which just happened to have its 8th Anniversary today. I got a really nice bread pudding for dessert for free today. They had a special wine (which i had to return because it wasnt at the right temperature, dont want cold wine when its snowing outside), the Sangiovese was a little better but I didnt complain after that. Pizza was gorgonzolla and capers. My 'usual' was good but did not like black olives, cucumbers and yellow squash in it. Have to let them know not to add that, though pine nuts are a good addition.

    Just finished the second season of Six Feet Under, what a great season. I am so getting hooked on to it. I havent worked out today and dont feel good about that but I am so bloated, I dont feel like moving today, maybe its going to be an early night after I finish my tax consultant role play with Mauli.

    International Museum of Women

    Here is something I found interesting in my mailbox today:

    The following online exhibition, launched by the International Museum of Women this month. The aim of the "Imagining Ourselves" exhibition  is to connect together women from a particular generation (20-40 years of age). To this end, it features essays, poetry, stories, music, fine art and photography from women in this age group and allows browsers to add their own comments and/or writing to each piece if interested. Many of the featured pieces seem to have catalyzed a storm of comments from all over the world.

    More power to women! Now to me (afterall dis ma w'ld) :-) I am 750 bucks and a certificate richer! thats right, my award much to my surprise was not only a plague but a nicely typed up appreciation letter, 'Difference is you' award of a whooping 750 bucks, and they had an audience (all company staff meeting) with applause and our president present it. I guess it was a bigger deal than I thought it was. This ofcourse in addition to a raise and everything.

    Today wasnt too bad a day, meeting with an integrated group of people on downtime procedures, then grading customer tests where I had to fail 3 of 4 people and call them and tell them they have failed me was the tough part. After that another meeting which was scheduled for 3 hours but finished in 1.5 and I ended up working from a starbucks near by and waiting for our staff meeting. We had some interesting presentations about one of the major exhibitions (which i should have been called to but wasnt) and our booth and the competitor booths. It was rather educative.

    Got back and managed to workout, ran for about 1.5 miles up the hill (incline 2.0, no way I am running outside, its still 40 degrees F)  and did some shoulders, think I am going to concentrate on strength training again. It feels like I have lost a lot of muscle in my little atkins experiment and everything I gain now is fat which makes me feel like shit. Saw an interesting episode of LOST today, wasnt too surprised, I knew Sun was pregnant and I knew Ana Lucia was going to find the balloon. Other than that there wasnt much, but it was still interesting to see an episode after so long. Another one next week! I am interviewing a girl tomorrow, should be something :-)

    Yesterday was 'zara hatke'

    Zara Hatke reminds me of those interviews that bollywood directors, actors and writers would give, they always used this word. Every movie, a copy of every other movie will always be Zara Hatke. Recently Rids gave me an example of an actress saying ' Yeh film Zara Hatke hain, maine is me 1/3 length ke skirt pehne hain instead of 1/2 length'. Made me ROFL on that for a while :-)

    I had a long morning with phone calls and boring meetings. I forgot my powercord at home, so my laptop was going to die by around 11.30ish. So I decided to go home and on my way decided to not return till 5 pm which was when my other meeting was. When I reached, I changed into workout gear and went for a run and then workout till 1 pm. After that heated up some leftovers from pfchangs and watched 2 hours of 6 feet under till 4 pm (ofcourse stopped it around 10 times to reply emails, hence 2 hours of tv in 3 hours). It was very refreshing.

    Meeting till 6 pm, went to H&R block of my taxes yesterday. Apparently it really helps if you tell them the tuition you paid, gives you about 20% back. So I had to come back to the hotel room to get the exact amount from my external hard disk which was ofcourse documented in my very well kept monthly accounts for the last 5 years ! It was a good idea to come back because I found some coupons online after H&R block and saved around 50 bucks :-)

    Cause for celebration, went to Abuelos which is a place just opened up next to PF Changs. From their website:

    Today's been a rather ordinary day but I guess I am going to get some kind of award tonight so I am looking forward to that... aight now back to grading my customers tests, they are pretty anxious about it!

    7 Reasons Not To Mess With A Child

    Its been a long boring morning, herez something that cheered me up! I love the fifth one, reminded me of a guy Vishal Sablok in my class (I thru X) who would have said something like this :-)

    A teacher was giving a lesson on the circulation of the blood. Trying to
    make the matter clearer, she said, Now, class, if I stood on my head, the
    blood, as you know, would run into it, and I would turn red in the face..

    blood doesn't run into my feet

    The children were lined up in the cafeteria of a Catholic elementary school
    for lunch. At the head of the table was a large pile of apples. The nun made
    a note, and posted on the apple tray

    large pile of chocolate chip cookies.

    Boast of Quietness

    I got an email from one of my forums about a reading by Kiran Desai in Boston. She started off with this poetry, when I read this it suddenly touched a cord inside. My favorite line is in italics

    Biscopewallahs

     Bioscopewallahs where the people who performed a live show – showing “pictures” or “photographs” in a box, entertaining children. Children use to peep through the small circular openings (usually four) in front of a wooden box and the performer use to sing or give a live commentary usually about the pictures.

    This was probably the most interesting email I received today! Imagine what a boring day it might have been. A very very ordinary day with work from 9-6, dinner at PF Changs and emails and expenses till 10 pm.

    No interesting conversations besides some talk at dinner with Joe regarding his football career and surgeries and what not... hopefully tomorrow will be better.

    Imperishable Body of a Russian Monk:
    Hambo Lama Itighelov, a spiritual Russian Leader, passed away in 1927. Per his request, he was first exhumed in 1955. When after the third exhumation in 2002 after 75 years since the Lama's death, his body still showed no signs of decay, medical experts decided to examine the miracle. The grave contained a wooden box and there was a sitting Buddhist lama in a 'lotus' position; researchers reported that his joints flex, the soft tissues were elastic just like a living person, and there was even a pleasant fragrance upon opening the 75-year old grave! [ more ]

    Art Psychology

    Recently I have been really reading up on this field. When I studied Art History in Metz, France during my study abroad program in 2001, I was always more interested in finding out why the painting was done the way it was, what was going on in the artists life at that time. His/Her work helped me understand the person better. Recently I was able to do that, interpolate or you can call it merely speculate that with a friend of mine. Atleast she thought it was pretty insightful. Now it makes me think I should dig a little more in this field.

    Now a recap of last two days which to me seemed like one long day. Putting those curtains up and getting ready for Arpit's office party happened till around 3 pm, after which I left to meet Vishwas and Nitin at the Georgia Aquarium. Now that I have the toll card, I could go in the cruise lanes and not have to wait, cut my commute time in half. So I was there way earlier than I had planned, so the only place I knew to go from there was Octane. I have spent many many hours in this coffee shop. Its always great to go there, now apparently they have a little bar too. I had my usual Breve there, while I picked up Atlantan. Thats a really nice magazine, I am going to try and read that more often now.

    Georgia Aquarium was really amazing, specially the Beluga whale, the hugh 9 million gallon tank the size of a football field, the tropical fish tank, african penguins, jelly fish and sting ray. But the best thing by far were the dragon fish! they actually looked like drangons which are mythical creatures as far as i know! It was great.

    Then we took Nitin around GT campus, student center, commons, CRC, Tech Square. Met up with Joya at Barnes and Nobles. Went on with three of them to meet Sumier, Chirag and Amisha at Atlantic Station. Followed them to Sumier's place where we met Soumya and Rahul. Then we went to Phuket where we met Anjali and Bimal, Basically it was a very big group where we couldnt keep track of how we know so many people but it was fun. Later that night we went to Halo and were asked to join a Bachlorette part, had a few rounds of Patron shots, Citron shots, Midori Sour (my drink of choice that night) and a bottle of Fat Bastard Shiraz at home to finish the night.

    Must have slept at 4, got out at 9 and had some amazing food that Toral had prepared for the party last night. Lesson learnt: never have cheese sticks and jalapeno poppers without heating them! Off to the airport, connecting through Cincinnati to Milwaukee. Drove to Madison last night... and now back to work!

    Big Change of the day: I have decided to switch my glasses from thick black rimmed to Rimless frames :-)

Hobart is the second largest elementary school in the country and 90% of its students live below the poverty line.     V for Vhatever

    Just got back from this movie. This is one of those where you dont know when you walk out if you want to say I loved it or I hated it. It makes you think I will give it that, and as far as I think - I understood what the message of the visual novel was, not necessarily agree with the conclusions or methods behind the political statement but I understand it. It was tastefully done, had its moments, not something that I would like to own a dvd for but wouldnt might watching it a few times. It was an interesting movie, I agree to disagree with some aspects and the fact that it made me thing pretty much gives it a thumbs up in my books. Though I am not very surprised that Alan Moore the creater of the graphic novel disowned it, it kinda confused the audience about the message towards the middle where evey is tortured, something they could have done without. Overall I think it was worth a friday night at 9 dollars.

    Today had a crazy start, 7 am I get up with a shock. I didnt have the conference number for the 12.30 phone call with my customers, so I call in the emergency IT pager, to get that voice conference line. And that starts me with my phone conferences all the way till 2 pm. After that I was so tired, I went to sleep till 5. I missed around 5 calls by then, one was a very important one which I called back and found out I am getting an award from work and a raise of 12%. That was really exciting !

    Living in a home, friday evening was spent fixing up the room and the curtains. It looks beautiful, i have green curtains in my room. Yesterday though was a relatively relaxed day, went to CPK for lunch with Samedi. Dinner at Blue pointe with Rahul was fun too. I had a really nice Malbec with Gormet Soba noodles with asparagus and shittake mushrooms. The spicy wine went really well with my sweet noodles. The desert was great too, specially the chocolate moose cake.

    Spoke to both my managers today and they have told me not to work! Thats always a great feeling :-) I am pretty buzzed on Pinotage right now, so its time to sleep on my wonderful new bed and mattress.
